Registrations

We are only able to accept new patients living within our catchment area (see map page). All new patients are asked to complete a questionnaire about their past medical history and attend for a registration health check as part of the registration procedure. This is essential as it may take up to three months for your previous records to reach us. You can also pre-register online and then attend for a health check. You can do this by downloading an adult's registration form for new patients over 16 or a child's registration form for those under 16. Please inform us of any change of address or telephone numbers. Your children’s immunisation records are required as part of their registration process.
